This application, properly completed, will allow that your Ship Model can be properly screened and classified for inclusion as a permanent entry on the Registry site.  Please be as accurate as possible in your descriptive and Historic information.

In addition to the application form below, please email up to two additional images of the model to jamd@accesscable.net .  These images will be displayed along with the ship model details.  Please see that the photos are of a quality that will display well on the website.  Preferred image size is 1024 x 768 pixels and in JPEG format, however, larger images and other formats may be accepted.

A one-time fee of $20.00 (Canadian Funds - or $18.00 US funds) will be levied up front, when a Model is registered, and the fee will cover a permanent, transferable listing.  Payment is by cheque or money order and should be sent to:
